The OG Crew’s policy follows the charge by O.S.H.A. and other regulations to provide a safe and secure working environment to all employees. One aspect of that policy is to attempt to ensure that all employees are drug free and not under the influence of any drug or alcohol while working. The OG Crew requires all employees to sign the following authorization to perform dug screens as necessary to aid in providing a safe work environment for all.
I agree that The OG Crew requires the administration of drug test at the time of employment, on the job accident and/or any other incident that relates to the safety of any OG employee, or the safety of the general public exposed to any unsafe act on an employee’s part. I also understand and agree to perform a drug screen at any time my work performance or appearance may suggest I may be under the influence of alcohol or drugs.
If at any time a drug screen has a positive result, OG has the right to Terminate, or to discipline accordingly. OG’s Drug & Alcohol policy is available at any time for any and all employees to read. Anyone wishing to obtain a copy may request one.

I understand that employment with OG is based on mutual consent, and both OG and the Employee have the right to terminate employment with or without cause, at any time.
I understand that should my employment with OG be terminated by myself or by the Company, I will return immediately any/ and all Company property (tools, keys, hard hat, safety glasses, etc).
I also understand that if I do not immediately return all Company property, OG will withhold from my last paycheck the replacement cost for such items not returned by Employee.

I have received information that informs me how to get health care under my employer's workers' compensation insurance.If I am hurt on the job and live in a service area described in this packet, I understand that:
• I must choose a treating doctor from the list of doctors in the network. Or, I may ask my HMO primary care physician to agree to serve as my treating doctor.
• I must go to my treating doctor for a health care for my injury. If I need a specialist, my treating doctor will refer me to a specialist. If I need emergency care, I may go anywhere.
• Texas Mutual will pay the treating doctor and other network providers for the treatment for my compensable injury.
• I may have to pay the bill if I get health care from someone other than a network doctor without prior network approval.
Knowingly making a false workers' compensation claim may lead to a criminal investigation that could result in criminal penalties such as fines and imprisonment.
